Months have passed since the gangsters and the men from The Base ventured to the island of horrors to rescue their loved ones stolen from them.

Abandoned after the rescue, the island has been taken over by human sex traffickers and turned into a “holding cell” for trafficked victims awaiting auction. Agent Alvarez and his band of rogue agents follow a lead to the rock of horrors just off the coast of Costa Rica. The former nightmare of The Island has grown, and this time, a lone team may not be enough to take down the ringleaders and rescue the innocent trapped in the bowels of the horror show.
